Junior Doctors are due to strike between the 2nd October 2023 and 5th October 2023. This may mean that there will be a reduced amount of appointments during this time. New Patients

Anyone living in Wainscott, Cliffe Woods, Cliffe, Higham, Upnor and surrounding areas (please see the section on Changes to the Boundary) are welcome to register with us for medical services. All of our sites have disabled access.

To check if you live within the practice catchment area please click on the following:

If you have any doubt, please contact the practice and speak to a member of the Clinical Administration Team who will be able to inform you whether your address is within the practice area.

We will register you with a named doctor but you can see any doctor at any of the sites.

The receptionist will give you a form to complete to give us some basic information about you. When submitting your application it is really important to ensure you have completed all of the boxes on the application form. Incomplete forms will not be accepted until all the necessary information is provided.

Please allow 2-3 working days from when returning the completed forms to allow the registration process to be completed before booking an appointment. The practice is unable to offer you an appointment until you have been registered.

Please download and complete the registration form and return to any of the Highparks surgeries.

Please note we also require current proof of address in the form of a bank letter, council tax bill or some other official document and proof of ID in the form of a passport or driving license,for children please provide a copy of their red book.

We aim to complete all registrations in a timely fashion but please be aware failure to supply the above documentation could result in a delay.

Changes to Personal Details

Please advise us immediately of any changes to personal details such as name, address or contact details. You can do this by clicking on the ‘Update your details’ tile on the front page and submitting it via the website.

From 1st April 2015, all patients registered at any GP practice are required to be allocated a named accountable GP.

What does ‘accountable’ mean?

The named accountable GP is responsible for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each patient as required. However, this does not mean that they will be the only GP or clinician who will provide care to that patient. If you would like to know your accountable GP, please ask at reception.
