Non-NHS Services

We are able to perform a number of non-NHS, or private, services.

These include:

• Private Sick notes
• Malaria prescriptions
• Letter of Medical Fact
• Short letter of opinion
• Short medical form
• Childminder health form
• HGV/Taxi/Pre-employment medicals
• GP Reports
• Formal Assessment of Mental Capacity
• Formal Adoption Medicals.

Please ask at reception if you require further details/information regarding fees.

From the 1st April 2024 the practice will no longer be able to receive cash payments for non NHS services.  However the practice does have the facility for card payments to be made via a credit/debit card (excluding AMEX) or by cheque, which should be made payable to ‘Highparks Medical Practice’.
