Travel Vaccinations
Go to your local surgery and ask for a ‘Travel Vaccination Form’ or print using the following link: Travel Vaccination Form
Fill in the form and return it to your local surgery and ask reception to book a travel appointment with the nurse.
The nurse will tell you which injections you need for your trip and will give them to you at the appointment. Some vaccines incur a charge.
Travel clinics are held every three weeks at our Cliffe surgery.
