Register with our practice
People who live nearby can sign up with our surgery. All our buildings are easy to get into if you need help.
We will give you a named doctor, but you can see any doctor at any of our surgeries.
The receptionist will give you a form to fill in. All the boxes must be completed or we cannot accept it.
After you return the form, please wait 2–3 working days before booking an appointment.
You will also need to bring:
- Proof of where you live, like a bank letter or council tax bill
- Proof of who you are, like a passport or driving licence
- For children, a copy of their red book
You can register by;
- Collecting form from any of the surgeries
- Printing the forms and bringing to any of the surgeries: NEW PATIENT REGISTRATION FORM – PRF1

Catchment Area (Practice Boundary)

Since 1 April 2015, everyone who is signed up with a doctor’s surgery has a main doctor called a named GP.
This doctor helps keep track of your care and makes sure you get the right help when you need it.
You can still see other doctors or nurses, not just this one doctor.
If you want to know who your main doctor is, you can ask at reception.
Accessing someone else's information
What is Proxy Access?
Proxy access means letting someone you trust help you with your doctor things.
They can:
- book appointments
- order medicines
- talk to the surgery
- see test results or vaccinations
- help look at health records
How to AskYou can:
- ask to help someone else, or
- choose someone to help you
Fill in a form:
- Online – please complete this form
- Print form – Proxy Access Registration Form and bring it to the surgery with ID.
- We will reply within 21 days.
Online Services (Patient Access)
You can also use online services to:
- book or cancel appointments
- order repeat medicines
- see your health record
To register:
- online requests – use this form.
- Print this form Patient Access Registration Form return to the practice with required ID.
Once submitted, the practice will process the request and respond within 21 days.
